This facility is a member of the National Health Service Corp. As a National Health Service Corps Site, we promise to:

  • Serve all consumers
  • Offer discounted fees for consumers who qualify based upon a sliding fee scale
  • Not deny services based on a person’s ability to pay, race, color, sex, national origin, disability, religion, sexual orientation, or gender identity
  • Accept insurance, including, but not limited to: Medicaid, Medicare, Children’s Health Insurance Program (CHIP), and Peachcare

Sliding Fee Scale

Community Service Board of Middle Georgia serves all individuals regardless of ability to pay and offers a sliding fee scale to the uninsured or underinsured. Fees are determined by income or family size.

To qualify, please bring ONE of the following to your appointment:

  • Wage statement from the Department of Labor
  • Two most recent pay stubs
  • Documentation from Social Security
  • Official documentation regarding your wages or inability to
    work

* Please note that payment or co-payment is expected at the time of service. Inability to provide financial information or required payments can result in a discontinuation of services. If you are having trouble making payments, contact us to work with you on a case-by-case basis.
