The philosophy of Community Service Board of Middle Georgia is built on a fundamental belief in service delivery. When we deliver services that promote and protect valued lived experiences, those served are better supported and able to live meaningful lives within their community.
We further strive to provide easy access to high-quality, recovery-focused care. Recovery occurs through many different pathways. It is our privilege to instill hope, optimism, and a belief in recovery for those we serve, no matter the chosen path.

CSB of Middle Georgia is the recipient of numerous grants and awards, including a federal Substance Abuse Mental Health Services Administration Community Mental Health Center award in 2021, a SAMHSA Certified Community Behavioral Health Center Planning, Development, and Implementation award in 2022, and the State of Georgia’s Department of Behavioral Health and Developmental Disabilities’ 2019 Provider of the Year Award.
About Denise Forbes, MS LPC CMAC, Chief Executive Officer
Denise is a Licensed Professional Counselor and Certified Master Addiction Counselor with a distinguished career in the behavioral health field spanning four decades. She received both a Bachelor of Arts in Psychology, Summa Cum Laude, and a Master of Science degree in Psychology from Georgia College in Milledgeville, Georgia, now known officially as Georgia College and State University. She has served as CEO since March 2012 and promotes a strong teamwork atmosphere and work culture steeped in recovery-based practices with a high expectation of excellence in service provision and care. She is a staunch behavioral health ally and advocate and encourages input from individuals, family members, stakeholders, and staff as she leads the agency using a collaborative management style. According to Forbes, “It is the privilege of a lifetime to be a part of this exceptionally talented and dynamic team who give their all on a daily basis to ensure that the behavioral healthcare needs of our communities are met.”
