Moderate Customized Care Coordination (MC3)

MC3 focuses on building a sense of hope and future for families at a moderate range of need. MC3 principals are embedded within all outpatient services to build understanding and agreement based on continuously engaging families around what matters and understanding what better looks like. MC3 builds a sense that things can work out, reaches agreement on family goals, and puts a plan into motion. MC3 builds communication and collaboration with all agencies, community, and natural support involved and customizes our response to best move toward a sense of hope and future.

Behavioral Health Services for Young Persons and Their Families

Children, youth, emerging adults, and their family members can find hope and healing at CSB of Middle Georgia. Our services are available to young individuals, no matter their challenges.

Children, Youth, and Emerging Adult Substance Abuse Services

Children and young adults struggling with substance abuse or misuse can find recovery and learn how to manage feelings without using dangerous substances.

Sliding Fee Scale for Young Adults 18 and Over

We serve all individuals regardless of ability to pay and offer a sliding fee scale to the uninsured or underinsured. Fees are determined by income or family size.

To qualify, please bring ONE of the following to your appointment:

  • Wage statement from Department of Labor
  • Two most recent pay stubs
  • Documentation from Social Security
  • Official documentation regarding wages or inability to work

*Please note that payment or co-payment is expected at the time of service. Refusal to provide financial information or required payments may result in a disruption of services. If you are having trouble making payments contact us to work with you on a case-by-case basis.
