Community-Based Mental Health Services
Through groups and programs, individuals can find further healing and stabilization within their community. Our services include a Community Support Team, Case Management, Intensive Case Management, and Psychosocial Rehabilitation-Individual.
Community Support Team (CST)
Those experiencing severe mental illness are paired with a Community Support Team to develop beneficial skills for recovery and daily function. The Community Support Team is led by a licensed clinician and serves the individual with resources to help decrease hospitalizations, incarcerations, emergency-room visits, and crisis episodes. The team offers support to increase community tenure, independent functions, vocational work, socialization, personal satisfaction, and autonomy.
Psychosocial Rehabilitation – Individual (PSR-I)
Psychosocial Rehabilitation-Individual services provide individuals with rehabilitative skills building essential to improving an individual’s functioning. PSR-I also teaches learning skills to strengthen the ability to self-access necessary services. Additionally, this service creates environments that promote recovery and support emotional and functional improvements.
Intensive Case Management (ICM)
For those who need additional help navigating growth and recovery, Intensive Case Management provides individuals with services related to:
- Behavioral care
- Education
- Finances
- Housing
- Medical care
- Socialization
- Vocation
- Wellness
ICM aims to decrease hospitalizations, incarcerations, and episodes of homelessness while increasing housing stability, employment, and community engagement.
Case Management
Through case management, individuals receive environmental support and care coordination essential for growth and improvement. Case management helps the individual improve functioning, gain access to necessary services, and create an environment that promotes recovery.
How to Get Started
Individuals must undergo intake at an outpatient clinic to receive a referral to community-based services. Appointments and walk-ins are accepted.

Insurance
We accept most forms of insurance, including:
- Amerigroup
- CareSource
- Centene/Peach State
- Medicaid
- Medicare
- Private/commercial insurance
- Uninsured
Sliding Fee Scale
We serve all individuals regardless of ability to pay and offer a sliding fee scale to the uninsured or underinsured. Fees are determined by income or family size.
To qualify, please bring ONE of the following to your appointment:
- Wage statement from Department of Labor
- Two most recent pay stubs
- Documentation from Social Security
- Official documentation regarding your wages or inability to work
