The Power of Follow-Up Care: Why Outreach Matters

Following up with individuals after discharge is more than just a courtesy—it’s a vital step in supporting their recovery and well-being. Research shows that the level of post-discharge outreach directly impacts attendance at aftercare appointments, which are crucial for continued progress.

Without any outreach, attendance at follow-up appointments ranges from only 28%–32%.

With at least one outreach attempt—even if unsuccessful—attendance rises significantly to 42%–47%.

When a successful connection is made with the individual after discharge, attendance increases further to 47%–53%.

These numbers clearly demonstrate that outreach makes a measurable difference. Every call, message, or in-person connection is an opportunity to encourage engagement in ongoing care, helping individuals stay connected to the resources and support they need to thrive.

By making follow-up a standard practice, we can improve outcomes, strengthen relationships, and show those we serve that their recovery journey matters—long after they leave our care.
